Chicago Med's heartbeat is holding steady after 11 seasons of medical emergencies, impromptu surgeries, and the occasional ED dustup.
Dick Wolf's NBC medical drama, a companion to Chicago Fire and Chicago P.D., celebrated its 200th episode this past October and wrapped the first half of its 11th season on Wednesday, Nov. 12. The midseason finale is reliably packed with compelling cases, but ups the ante when Dr. Caitlin Lenox (Sarah Ramos) leaves the hospital and finds herself at the center of a terrifying domestic dispute amid a torrential storm.
How does "Double Down" set the table for the back half of the season? Let's dig into the episode — and its multiple cliffhangers.
Who is Devin Carter?
"Double Down" sees the return of Devin Carter (Jack Falahee), who we last saw in the season 11 episode "Found Family." In that episode, Lenox learns that Devin has been abusing his wife, Faye (Olivia Nikkanen), after she comes into the ED with burns and a lacerated spleen. Lenox offered help to Faye, but she still went back home with Devin.
In "Double Down," Lenox is shocked when Devin turns up in the ED. Intoxicated, he passed out in a pharmacy while covered in wounds, including bloody knuckles. A concussed Devin claims he was injured in a hockey game, but Lenox and Ripley (Luke Mitchell), who also witnessed Devin and Faye's previous trip to the ED, are concerned for Faye's well-being.

Yes, but she's not in good shape.
Lenox, worried the raging storm would prevent the police from performing a wellness check, sets out to Devin and Faye's home on her own, asking Ripley to keep Devin in the ED. The house appears empty, but after Lenox spots a broken bottle on the ground, she takes it upon herself to break in and investigate.
She finds a bloodied and battered Faye at the bottom of the basement stairs with a broken ankle. Unable to get a signal in the basement, Lenox goes back upstairs to call an ambulance, only to be attacked by Devin, who managed to elude Ripley and leave the hospital on his own.
This is how the episode ends, with Lenox and Faye both subdued by an armed (but still injured) Devin.
Does Jeremy survive his surgery?

The heart of the episode lies in Jeremy (Zach Appelman) and Esme (Tetona Jackson), a couple we saw earlier in the season who are expecting a baby daughter. Unfortunately, Jeremy's cancer isn't responding to his latest chemo regimen, meaning he has just weeks to live and won't see the birth of his child.
Esme pleads with Dr. Asher (Jessy Schram), who is also pregnant, for a way Jeremy could meet their child before passing. It's still too early to induce, but prolonging Jeremy's life could help him survive until it would be safe.
Dr. Archer (Steven Weber), the father of Asher's baby, helps develop a plan to remove Jeremy's tumor and execute a procedure that would heat his chemo. Hot chemo, Archer explains, has a better shot of penetrating and killing cancer cells, thus giving Jeremy more time. The surgery, however, is very risky, with a large chance that he wouldn't survive. Still, Jeremy and Esme agree to make the gamble, despite it meaning Jeremy will spend his final days bedridden and in pain. Meeting his baby is the most important thing to him.
The surgery appears to be going well, but the storm knocks out the power during the procedure. Even worse? The backup generators aren't turning on. As his blood pressure decreases, the chance of Jeremy dying on the table increases.
So, does he survive his surgery? We won't know until the show returns next year.
Who was right about Angelica — Dr. Charles or Dr. Rabari?

At first? Neither. In the end? Both of them. Let's break it down.
Angelica, a teenage gymnast, shows up at the ED with a broken ankle. She's been feeling dizzy lately, and she and her mother are wondering if it's due to her new anxiety medication, which is still in the trial phase. As luck would have it, one of the doctors shepherding it along is Dr. Rabari (Manish Dayal), who's at Gaffney.
But when Dr. Charles (Oliver Platt) posits that the medication could be causing Angelica's symptoms, Rabari is resistant to taking her off of it, lest they disrupt her treatment. His behavior is odd, and Charles begins to suspect that Rabari would rather put Angelica in danger than see the future of his new drug compromised.
Charles becomes particularly spooked when Rabari seems to randomly order a vitamin A test for Angelica. The results show that Angelica has toxic levels of vitamin A, which is rare but could have any number of causes — including performance-enhancing drugs. Charles, though, fears that Rabari spiked Angelica's IV in order to protect his own drug. Tensions flare between the two.

In the end, however, the answer isn't so sinister. Angelica routinely covers herself in glitter when she performs, using products like hair gel and body butter to apply it. This leads Charles to inspect the product labels, which reveal that they are brimming with vitamin A. It was the over-application of those products that caused Angelica's vitamin A spike.
Rabari is furious that Charles questioned his integrity, and Charles apologizes. Rabari accepts the apology and gives Charles credit for discovering the cause. "Maybe we're a team," says Charles.
Do Frost and Howard get together?

Not yet, but we haven't lost hope.
Earlier in the season, Dr. Frost (Darren Barnet) asked out Dr. Howard (Ashlei Sharpe Chestnut). She turned him down, saying she doesn't date coworkers. Secretly, though, she's carrying a torch. When she speaks with Asher about it, she tells her to get over it and make a move. Asher, who's feeling overwhelmed by Jeremy and Esme's predicament, says we should "do the things that make us happy while you still have the time."
Frost, for his part, is already moving on. In a brief Chicago Fire crossover, we see Frost flirting with paramedic Lizzie Novak (Jocelyn Hudon), who responds to his request for her number by writing "9-1-1" on his arm.
Still, Frost is struck when Howard later reveals her feelings for him and admits that she creates obstacles in her personal life to keep things uncomplicated.
"We missed our window," she tells him. He asks why she's bringing it up now, and she says because she "needed to."
